Contact us and make Appointments

You can contact us to make an appointment or click the link above to make the appointment online. Please call or text if your desired time of appointment is not available on the website, we can try to manually fit you in to the best of our ability.  

After making the appointment, an email with a link will be sent to your mailbox for you to complete a couple of forms on a HIPAA complaint and secured platform. You may complete the necessary forms at home or arrive at the office 20 minutes early to complete them.
